Client Consent & Waiver Agreement

By booking and attending an appointment, you acknowledge and agree to the following:

• You have disclosed any relevant medical conditions, medications, or recent procedures
• You are not currently undergoing or recovering from radiotherapy, chemotherapy, or pregnancy-related medical treatment (unless otherwise approved)
• If booking a massage, you confirm that you are not currently pregnant
• For SPMU (Semi Permanent Makeup) you are not currently or recently taking any blood thinners AND are not taking the Manjaro injection/medication.
• You understand that results may vary and that no outcome is guaranteed
• You release the practitioner and business owner from liability for any unforeseen reactions or complications, except in cases of proven negligence
